The success stories behind penis enlargement surgery appear to be few and far between. Whether this has more to do with the taboo nature of saying I got a cock-job, and my new equipment is great than a poor success rate is up in the air. There are a number of "penis enlargement professionals" with wide-reaching Internet marketing campaigns. The surgical penis enlargement industry has not yet received the backing of the American Medical Association, and while not prohibited, is not well regulated. Most surgeries do not have any complications, but a surprisingly high number (4-7%, depending on the source) have some sort of unforeseen negative effect, including heavy scarring, deformities of dermal grafts (fat injections moving around), or a reduction in the body's ability to have an erection. Yes there are ways to alter the size and appearance of the penis through surgery. The medical term for these procedures is collectively known as phalloplasty.
Penis enlargement surgery usually involves either a single procedure or a combination of two. Suspensory ligament removal increases the visible length of the penis. Fat transfer increases the girth or circumference of the penis. A single procedure costs approximately $5000. Dual procedures cost about $8500. Penis enlargement surgery does not involve insertion of any prosthesis or foreign objects. Instead, the procedures use the body's own materials.
The penis is longer than you may think. A portion of the penis, 1.5-2 inches, lies inside the body. During suspensory ligament removal, the ligaments which attach the penis to the pelvis are severed. This allows part of the inner portion of the penis to descend. This newly exposed portion of the penis of course has no skin on it, so the surgeon advances skin from the lower abdomen down to cover this section.
After the ligaments are cut, the patient may be instructed to wear weights to help pull down the internal portion of the penis. If all goes well, the patient should gain between .5 - 1.5 inches in external length. There will be some other, not so great, changes. We will discuss these a little later.
What's length without width? Many men who undergo suspensory ligament removal also have the girth of their penis thickened. The surgical procedure to increase penile girth is fat transfer. Fat cells from one area on the body, usually the lower abdomen or buttocks, is removed and transferred to the penis shaft. The circumference increase is approximately 18%. Don't count on maintaining this increase though. We will explain why in the next section.
Penis enlargement surgery, phalloplasty, is considered a form of cosmetic surgery, but it is not taught in any medical school. The procedures are unregulated and considered experimental. The American Urological Association (AUA) and the American Society for Aesthetic Plastic Surgery (ASAPS) have denounced penis enlargement surgery as being neither safe nor effective. These medical associations also discourage their members from offering and performing the surgery. This is because of low success rates and high rates of risk and damage to the patient.
The number of men who are disfigured after enlargement surgery is so high that some physicians devote their entire practice to repairing such damage. Though certain enlargement surgeons claim that the surgery is a safe, painless, one-time procedure, the truth is quite different. In fact many enlargement surgeons are being sued for causing deformity, dysfunction, and false advertisement of the results and risks of surgery.
Even if you avoid any complications or scaring, the fat transfers will likely need to be redone to correct a bumpy appearance. Meaning that you will need several surgeries to maintain a normal looking penis. Whether it is to correct the work of one surgeon or to refresh the original results, you will probably undergo several procedures.
In a recent article in the Journal of Urology, physicians stated that enlargement surgery carries such risks that it should not even be considered unless a man's erection was less than 3 inches. Our suggestion is to take their advice. It is better to have an average or small penis that is normal looking and fully functional, than risk ending up with a lumpy, bumpy, hairy penis that points straight down.
We know that some men will choose this procedure no matter what medical boards say.

If you are still interested in the procedure, you should take certain steps before going through with it. Here is what we recommend...

1) Investigate. Gather all the information you can from reliable sources, non-partial sources.

2) Find a board-certified surgeon with lots of experience performing penis enlargement surgery. Different physicians have different skill levels and use different techniques. Some cosmetic surgeons are more successful at this surgery than others.

3) Talk with the surgeon before paying for and undergoing the surgery. If your surgeon and his staff ignore your questions and concerns or fail to address your concerns, leave and find another surgical team.

The ability to surgically increase penis size has been advertised as a "dream come true". Unfortunately, the results of the surgery can be more like a nightmare. One man who underwent the surgery stated the following in a Times article...

Many men who believed that surgical penis enlargement could enhance their penis, have instead suffered painful, and disfiguring results. Not all penis enlargement procedures leave the patient heavily scarred, but certain unwanted changes will occur in all patients.
Because the skin from the lower abdomen is pulled down to cover the newly exposed length, that portion of the penile shaft will be heavily covered with hair. Also, since the ligament that attaches the penis to the pelvis is cut, your erections will forever point downward.
The fat transfer also has the complication of not being permanent. This is because fat cells injected into the penis are not properly anchored. Therefore, the fat cells often reabsorb or clump together, giving the penis a strange lumpy appearance. These are the unavoidable results of penis enlargement surgery. There are other potential risks with this surgery. We list them below in order of greatest frequency.
1) Penis shrinkage. Enlargement surgery often has the reverse effect, leaving men with smaller penises than they started out with. This is caused by scar tissue. In the majority of cases when the suspensory ligament is cut, scar tissue forms. The scar tissue develops as a natural attempt by your body to heal the ligament. However, this scar tissue causes the penis to retract and greatly shorten.

2) Infection. All surgeries carry a risk of infection. But the rate of infection after penis enlargement surgery is far higher than normal. This is partially due to brief post-operative care which is usually only 2-3 hours. Other factors include poor patient education by the surgical staff and sanitary conditions in some surgical rooms.

3) Deforming scars. In addition to scar tissue that forms where the ligament was cut, some men end up with scars along their penis shaft. A large number of men have unnatural, scarred, and disfigured penises after enlargement surgery.

4) Impotence (Erectile Dysfunction). During surgery, the physician has to be extremely careful not to damage any of the blood vessels or nerves that supply the penis. In case you did not know, the penis has a large number of both. If a mistake is made and either a blood vessel or nerve is damaged, the result could easily be impotence.
